Students New to Illinois
· A physical examination on an Illinois Physical form within one year prior to the first day of school with updated immunizations is due by October 15, 2023.
· A vision examination by an eye doctor on an Illinois Eye Exam form is due by October 15, 2023.
· A dental exam on an Illinois Dental Exam form dated between November 15, 2022, and May 15, 2024, is due by May 15, 2024.
Kindergarten Students
· Physical exam on an Illinois Physical form within one year prior to the first day of school with updated immunizations is due by October 15, 2023. Please be sure a parent/guardian completes and signs the Health History portion of the physical before it is turned in, as the physical is not valid without this information.
· A vision examination by an eye doctor on an Illinois Eye Exam form is due by October 15, 2023.
· A dental exam on an Illinois Dental Exam Form dated between November 15, 2022, and May 15, 2024, is due by May 15, 2024.
2nd Grade Students:
· A dental exam on an Illinois Dental Exam Form dated between November 15, 2022, and May 15, 2024, is due by May 15, 2024.
Health Forms: If your child has been diagnosed with a life-threatening allergy or asthma, please be sure to complete the required health forms and turn them in to the health office with any medication ordered as soon as possible. Medications During the School Day: Any medication required during the school day, even over-the-counter medications, requires a D203 School Medication Form to be completed by a physician and parent/guardian. This form can be found at https://www.naperville203.org/Page/2384
