Oral Health Examinations - Dental exams are now required by the State of Illinois. As of July 1, 2005, a new State of Illinois law went into effect requiring that all students in Kindergarten, second and sixth grades have an oral health examination performed by a licensed dentist. For this school year (2006-07), the exam must be performed sometime after November 15, 2005, and before May 15, 2007.
 
Following your child’s exam, please have your dentist fill out the form required by the State (available by clicking here) and return it to the Health Office of his/her school no later than May 15, of each school year. Exam forms are also available there.
 
In addition, if you need a referral to find a dentist or financial assistance to obtain your child’s exam, please contact your school’s Health Office for assistance from the nurse.
